Reentry Transition Services
Lutheran Social Services of Northwestern Ohio (LSSNWO) provides re-entry case management services in both our Toledo and Fremont offices to those who are returning to the community after incarceration.
The re-entry case management program helps justice-involved clients by:
- Helping clients to obtained vital records necessary for employment, housing and public assistance.
- Helps clients with transportation assistance by providing bus passes and helping those who qualify to enroll in Medicab services
- Aids clients in finding and securing employment by connecting clients with second chance employers in the area, helping clients with resume building and applying for jobs
- Works to get clients housed by connecting clients with second chance housing and helping them to find rental, down payment, and utilities assistance for those who qualify
- Making referrals to substance use, mental health and court ordered services easy and cutting down wait times for these services.
